Services
We provide personalized emotional support, online support groups, and case management to help seniors and caregivers navigate challenges. Our services include care coordination, family meeting facilitation, and connecting seniors to essential resources.
Emotional Support
We provide weekly or bi-weekly visits to engage in meaningful conversations, activities, and hobbies ensuring that seniors feel connected and valued. Our trained staff offer one-to-one talk support sessions to help seniors navigate emotional challenges, cope with stress, and manage anxiety or depression that sometimes comes with changes in physical abilities or a new medical diagnosis. While we cannot provide medical advice our staff is trained to provide the support and coaching needed to help seniors get the best out of their doctors’ visits with our coaching support and doctor visit guides.
Case Management
Our case management services encompass bio-psychosocial assessments, PHQ-9 for depression screening, and as-needed support. This is ideal for seniors who have an existing care team but require extra social service assistance. We aid in coordinating their care, scheduling appointments, and providing service referrals, including meals on wheels, weekly grocery delivery, or connection with faith-based services.
Support Group
We facilitate online support groups for those seniors and caregivers who prefer to connect online, a safe and confidential place where seniors and caregivers can share their experiences, build friendships, and find comfort in knowing they are not alone.
Family Care Plan Meeting Facilitation
We offer assistance with care planning for your loved one and facilitate family meetings to ensure clear communication about their care needs.
How Does It Work?
Clients may request our services directly or through their Home Care Health Agency. Once we receive the referral, an initial intake process is conducted at a time chosen by the client with our trained social worker consultant. We then match your loved one with a consultant who best meets their needs. The consultant will assist with all chosen tasks, prioritizing them based on the client’s requirements. Support includes weekly or bi-weekly in-person visits, as well as phone calls or text messages, and connecting clients with local community resources when appropriate.

What we do NOT provide:
We do not provide support with activities of daily living such as bathing, dressing, feeding, toileting or walking support.
We do not provide a mental health diagnosis
We do not provide medication support
We do not provide legal advice or Nursing home placements.
What if my loved one’s health status has significantly deteriorated and needs to be placed in a long-term living facility? We can provide support through the application and referral process but cannot make specific recommendations regarding a facility and their requirements for admissions.
We do not provide 24/7 care
Our services are offered Monday-Friday between 9am and 5pm
